US Customs and Border Protection requires additional documentation for all international shipments valued above $2500 entering the United States. This form helps customs identify and process your shipment correctly.
Additionally, starting May 2nd, 2025, this form may be required for products manufactured in China or HongKong with a value exceeding 250$.
Why am I being asked to complete this form?
This is a federal government requirement for customs clearance. The form collects personal identification information that only you, as the recipient, can provide. For security and privacy reasons, neither Vestiaire Collective nor FedEx can complete this information on your behalf.
When and how will I receive this form?
FedEx will reach out to you after your shipping label has been created, but it may also happen a bit later while your parcel is in transit. FedEx will send emails to the email address associated with your Vestiaire Collective account. Please check both your inbox and spam/junk folders.
If you signed into your Vestiaire Collective account using Apple ID, please ensure to check your iCloud mailbox.
You'll receive 2 emails :
- An initial notification email
- A DocuSign email containing the CBP Form 5106 sent from the email address dse_NA3@docusign.net

You have the option to submit documentation proactively to FedEx (i.e. before FedEx sends the DocuSign link). Your tracking number will be provided to you as soon as your order is dispatched from our hub.
To do so:
- Download and complete the form available here
- Convert your completed form to PDF format
- Send it to: paperwork@fedex.com
Important :
- Subject line must contain ONLY the FedEx tracking number
- Leave the email body completely blank - do not include any text or additional information
What information will I need to provide on Form 5106?
You'll need to provide:
- An identification number :
- For US citizens, Green card holders, or individuals on working visas : Your Social Security Number (SSN) or Employer Identification Number (EIN)
- For non-US residents/foreigners: if you do not have a Social Security Number (SSN), you may ask to be provided with a CBP number via the form.

Is there a time limit to complete this form?
Yes, you must complete the form within 5 business days of receiving the email from FedEx. If the form isn't completed within this timeframe, your package may be returned to the sender. Once arrived at our warehouse, your item will be relisted under your account.
What happens after I submit the form?
Once you submit the completed form, the customs agent will process your information and release your shipment. This customs clearance process can take up to 72 hours. If your shipment shows no movement after 72 hours have passed, please contact us so we can investigate the situation for you.
Once customs clearance is complete, FedEx will deliver your package to your address. If additional information is needed, FedEx will contact you directly.
